National IAM Benefit Trust Fund Dental Plans

Effective January 1, 2017, the Benefit Trust Fund has consolidated the plans described below into four dental coverage plans.  Please read this Summary of Material Modifications for the details.

National IAM Benefit Trust Fund Dental Plans prior to January 1, 2017: 

The National IAM Benefit Trust Fund offers nine dental plans, which are administered by Delta Dental of Georgia. Covered employees and their eligible dependents may participate in a Benefit Trust Fund dental plan if it is negotiated into the Collective Bargaining Agreement.

Dental Coverage

Delta Dental has more than 142,000 participating dentists nationwide. It is the largest managed dental care network in the country. Seventy-five percent of all licensed dentists in the United States participate in Delta Dental's network. Benefit Trust Fund plans use both the Delta Dental PPO and Premier networks, giving plan participants a large pool of network dentists from which to choose.

The Benefit Trust Fund offers nine dental plans with varying benefit levels. Although it is simple to find a participating dentist, each plan has the option of choosing in-network or out-of-network providers.

Dental Services

The Benefit Trust Fund dental plans cover reasonable and necessary diagnostic, preventive, basic and major dental services. Orthodontia services are also covered by many Benefit Trust Fund plans. Services are covered at varying benefit levels depending on the type of service and the plan design. Before you visit any dentist, it is always a good idea to double check that the services you are planning to receive are covered by the Benefit Trust Fund dental plan, and that the provider participates in the Delta Dental PPO or Premier network.

Dental Summary of Material Modifications

Whenever a change is made to a dental plan, the Fund Office will send a Summary of Material Modification (SMM) to explain the change. SMMs should be retained in your Summary Plan Description for future reference. All changes made to a dental plan since the last printing of the Summary Plan Description will be included when the booklet is revised and a new edition of the SPD is issued. Obtain a Dental ID Card

Participants in a National IAM Benefit Trust Fund dental plan are sent a Delta Dental ID card when they enroll for coverage. You can also obtain an ID card by logging in to Delta Dental's website where you can create your own ID card and print it out at home. Or, you can request an ID card by calling Delta Dental at (800) 858-5252 ext 1615. Your card will have your member number and important contact information. Present your dental ID card whenever visiting a dental provider.
