Medical ID Card

Participants enrolled in a National IAM Benefit Trust Fund medical plan should receive a CareFirst Administrators medical ID card by mail.

Missing your ID card? Log in to Quantum Health to:

  • Print a temporary medical ID card
  • Request a replacement card
  • Request an additional card for a covered participant

 

Prescription Drug ID Card

Use your prescription drug ID card when filling a prescription at a participating pharmacy.
