The National IAM Benefit Trust Fund will develop plan proposals that you can use when negotiating collective bargaining agreements. To do this, you must first provide some information about the plans of interest and the potential group. Proposals will be provided within a few weeks after receipt of the following information:

Medical Coverage Information

  • Proposed effective date of coverage
  • Employer industry/type of business
  • Current schedule of benefits (a summary is sufficient) with plan changes for the last three years, if possible. If you would also like a plan comparison, an SPD would be helpful
  • Current premium rates or COBRA rates
  • Proposed renewal rate for the current year (including premium rates or COBRA rates for the prior three years, if available)
  • Employee census information, including age, gender, and type of coverage (single, family, etc.). 

Please be sure to include any employees who may have COBRA coverage or may be out on medical leave.

For Groups with Available Claims Data

  • Any known claims over $75,000 during the past 12 months (please provide details as to the nature of the illness, current status of illness, amount paid in claims thus far and if treatment is ongoing)
  • Monthly claims experience for the past 12 months (medical and pharmacy separated, if possible)
  • Monthly enrollment for the past 12 months
  • For groups over 100, please provide the above information for the last three years (if available).

Please do not provide names or any identifying information, other than the items requested above.

For Groups with No Claims Data

  • Each employee must contact the Fund to request and complete the National IAM Benefit Trust Fund’s Medical Profile Form.

Dental and Vision Coverage

The dental and vision coverage are pre-priced. The current rates are listed in the schedule of benefits for the National IAM Benefit Trust Fund Dental and Vision plans.

Short-Term Disability and Life and Accidental Death and Dismemberment Coverage

Short-term disability and life and accidental death and dismemberment coverage are available to groups that negotiate medical, dental or vision coverage into their collective bargaining agreement.

Short-term disability plans are pre-priced. The current rates are listed in the schedule of benefits for short-term disability plans found below.

Rates for life and accidental death and dismemberment coverage are determined based on employee census and the level of benefit requested. If you are interested in a quote for life and accidental death and dismemberment coverage, please contact the Fund office.
