National IAM Benefit Trust Fund Vision Plans
The National IAM Benefit Trust Fund offers two vision plans. Covered employees and their eligible dependents may participate in a Benefit Trust Fund vision plan if it is negotiated into the Collective Bargaining Agreement.
Vision Coverage prior to January 1, 2016:
With a Benefit Trust Fund vision plan, you can visit any qualified vision care provider you choose. There is no network associated with the Benefit Trust Fund vision plans.
There are no ID cards needed for a Benefit Trust Fund vision plan. Simply visit your vision care provider and receive care, then pay for your services and obtain an itemized receipt. Send your receipt to the Benefit Trust Fund along with a completed Vision Claim Form (PDF) for claims through December 31, 2015. The Fund will reimburse you for the cost of your vision care, up to the maximum benefit amount provided by the plan.
Vision Services
Benefit Trust Fund vision plans cover routine eye exams, lenses and frames. Contact lenses are also covered if purchased instead of lenses and frames. Benefits are payable once every calendar year. If you are reimbursed for covered vision services received in November, for example, you can be reimbursed for the same vision services the following January.
Participants in a Benefit Trust Fund vision plan are reimbursed directly for claims through December 31, 2015. The reimbursement rates for covered services and eyewear differ depending on the vision plan negotiated into the Collective Bargaining Agreement. Vision Summary of Material Modifications
Whenever a change is made to a vision plan, the Fund Office will send a Summary of Material Modification (SMM) to explain the change. SMMs should be retained in your Summary Plan Description for future reference. All changes made to a vision plan since the last printing of the Summary Plan Description will be included when the booklet is revised and a new edition of the SPD is issued.
